The resident trainee is expected to exercise honesty in logging in of his\/her cases. The e-logbook is a sacred document serving as an official basis for accreditation and credentialing\/certification, as stated above.<\/p>\n<p style=\"padding-left: 80px;\">b. The resident trainees should diligently enter the data of their case, completing the initial data required within a prescribed lock-in period. The lock-in period is set at two (2) weeks, meaning the case should be logged within two weeks after the case has been done. Failure to do means that case is automatically forfeited.<\/p>\n<p style=\"padding-left: 80px;\">c. The resident trainee should continue to update his e-logbook, especially as to the outcome and follow-up data. Once logged and locked in, updates may be done anytime.<\/p>\n<p style=\"padding-left: 80px;\">d. All the entries should conform to the Data Privacy Act, meaning there should be no information, especially in the entries indicated below, that can be traced back to the patient&#8217;s identity.<\/p>\n<p style=\"padding-left: 80px;\">e. The E-logbook is now being restructured to become an E-portfolio which is also an essential Workplace-based Assessment Tool. The data required in the e-portfolio includes the following:<\/p>\n<p style=\"padding-left: 120px;\"><strong>i. General Information:<\/strong><\/p>\n<p style=\"padding-left: 160px;\">MUST<\/p>\n<ol>\n<li style=\"list-style-type: none;\">\n<ol>\n<li style=\"list-style-type: none;\">\n<ol>\n<li style=\"list-style-type: none;\">\n<ol>\n<li style=\"list-style-type: none;\">\n<ol>\n<li>Patient ID including age\/gender and date of admission, and date discharged<\/li>\n<li>Surgery performed including the date it was done, appropriate CPT and Complexity<\/li>\n<li>Appropriate Classification systems<\/li>\n<li>Implants used<\/li>\n<li>The role played and members of the surgical team<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<p style=\"padding-left: 120px;\"><strong>ii. The E-logbook is an essential category for filling up the AC02 (Evaluation Sheet for Orthopaedic Training Programs), specifically the first MPL domain &#8211; Resident&#8217;s Surgical Skills \/ Institutional Clinical Material domain &#8211; and is given a maximum of four (4) points. The AC02a (Rubric for E-Logbook Assessment) is used as the basis and will be based primarily on the entries&#8217; completeness and accuracy. Scores in between numbers may be given depending on how the Trustee perceives the manner of usage.<\/p>\n<p style=\"padding-left: 40px;\">b. Surgical cases in accredited satellite hospitals of the said program are the only ones recognized by The Board. Should the program in question opt for additional satellites, this has to be cleared and approved so that the official e-logbook server can be given ample notice.<\/p>\n<p style=\"padding-left: 40px;\">c.
